Teenagers - hassle your ward or community based nurse or your social worker.  Ask them to fill one out an application form with you.  Get them to send it to us - we may be able to help you.

If they say they do not have one e-mail david@mattdotcom.org.uk and let him know.  Tell him the name and address of your nurse or social worker and he will send them the necessary papers.  He is good like that.
